RE: Kindergarten Registration Requirements
Per the State of Illinois, children entering Kindergarten MUST be five (5) years of age on or before September
1, 2024, and MUST have the following immunizations/physicals completed prior to the 1st day of school.
Please make arrangements with your physicians now so that your child(ren) will be ready to start their first day
of school on time. All required information will need to be presented in August for registration along with
proof of residency and certified copy of birth certificate.
Forms for the required immunizations and exams may be obtained at your registering school’s front office or
printed from the District 170 website,
Required VACCINATIONS
1. (DPT) Diphtheria, Pertussis, Tetanus
Four (4) shots are required
Last dose must have been administered AFTER the age of 4
2. Polio
Four (4) shots are required
Last dose must have been administered AFTER the age of 4
3. (HIB) Haemophilus Influenzae Type B
Zero (0)
Not required after 5th birthday
4. (PCV) Invasive Pneumococcal Disease
Zero (0)
Not required after 5th birthday
5. (HBV) Hepatitis B
Three (3) shots required
Last dose must have been administered AFTER 6 months of age
6. (MMR) Measles, Mumps, Rubella
Two (2) shots required
7. (VAR) Varicella
Two (2) shots required
Required PHYSICALS
A complete health physical, vision exam and dental exam must be obtained PRIOR to the first day of school.

Según el estado de Illinois, los niños que ingresan al jardín de infantes DEBEN tener cinco (5) años de edad el 1
de septiembre de 2024 o antes, y DEBEN tener las siguientes vacunas/exámenes físicos completados antes del
primer día de clases. Por favor haga arreglos con sus médicos ahora para que su(s) hijo(s) estén listos para
comenzar su primer día de clases a tiempo. Toda la información requerida deberá presentarse en agosto para
el registro junto con un comprobante de residencia y una copia certificada del acta de nacimiento.
Los formularios para las vacunas y exámenes requeridos se pueden obtener en la oficina principal de su
escuela de registro o imprimirlos desde el sitio web del Distrito 170,
